Physical Therapist Duties: 

Provide services to home health patients treating impairments, functional limitations, disabilities or changes in physical function and health status. Ensure the treatment and intervention provided is in accordance with the patient’s plan of care, accepted professional standards of physical therapy practice, applicable law and regulation, as well as applicable Trivium of New York, LLC policies and procedures  

Job Requirements Requirements:

NY State Physical Therapist License

Graduate of an accredited physical therapy education program or equivalent as recognized by the American Physical Therapy Association

One (1) year experience

Must be able to prepare and submit legible, relevant, and sufficient documentation

CPR certified

Valid Driver's license and active car insurance.
